Electors unable to sign their name due to physical incapacity
If, because of a physical incapacity, you are unable to sign your name, you may enrol by having someone complete and sign this form on your behalf. You must provide with your application a written certificate from a registered medical practitioner stating that you are unable to sign your name.
Assistance is provided if the polling official in charge of the polling place is satisfied that you are unable to vote without help. For further information please see Assisted Voting.
